Employees of USAble Life are insured for basic and/or voluntary group term life, and supplemental benefits through USAble Life. If your coverage terminates or has been reduced, you may continue coverage as outlined below.
You and your dependents may convert all or part of your basic and/or voluntary life coverage to an individual policy, without evidence of good health or medical examination, if your coverage stops for any reason other than the premiums were not paid.

Supplemental benefits may be continued on a direct bill basis. To ensure no lapse in your coverage, please submit two months premium with this application along with a voided check. Future premiums will be drafted from your checking account via electronic funds transfer (EFT). EFT automatically deducts the premium from your checking account each month. You must apply for portability and remit the first premium payment directly to USAble Life within 31 days from the date of the notification from USAble Life.
