Employees of USAble Life are insured for basic and/or voluntary group term life, and supplemental benefits through USAble Life. If your coverage terminates or has been reduced, you may continue coverage as outlined below.

Converting Basic, Voluntary Group Term Life, and/or Dependent Life

You and your dependents may convert all or part of your basic and/or voluntary life coverage to an individual policy, without evidence of good health or medical examination, if your coverage stops for any reason other than the premiums were not paid.

The amount you may apply for is the lesser of:

1. The basic and/or voluntary life coverage then in force; or
2. That part of the basic and/or voluntary life coverage which reduced.

You must apply for conversion and remit the first premium payment directly to USAble Life within 31 days from the date of the notification from USAble Life.

Porting Voluntary Group Term Life

You may apply for coverage under a portability policy for you if you elect continuation of insurance under this portability provision:

1. Your employment terminates for any reason prior to age 65; or
2. Your membership in an eligible class under the policy ends.

Portability is not available:

1. For individuals who do not meet the Eligible Person definition; or
2. Upon termination of the group policy.

If any of the requirements are not met, coverage may be continued through conversion.

Supplemental Benefits

Supplemental benefits may be continued on a direct bill basis. To ensure no lapse in your coverage, please submit two months premium with this application along with a voided check. Future premiums will be drafted from your checking account via electronic funds transfer (EFT). EFT automatically deducts the premium from your checking account each month. You must apply for portability and remit the first premium payment directly to USAble Life within 31 days from the date of the notification from USAble Life.
