REMINDER: If you are currently enrolled in an Accident, Critical or Hospital plan, please remember to file your Wellness Claim online using the Wellness Claim link in the left sidebar. Each plan pays you $30-$75 every year for completing an Eye Exam, Annual Physical, Cholesterol Screening, Mammogram, Pap Smear, Colonoscopy, Biopsy, etc. No proof documents are needed and there is no deadline to file. You are allowed one (1) benefit per policy, per covered person, per calendar year.

Voluntary Group Term Life (VGTL) insurance is the least expensive form of life insurance. Benefit is paid to the beneficiary when a covered individual dies, providing financial security in the event of an unforeseen death.

Medical Expense FSA allows you to pay for qualifying medical expenses on a pretax basis. Expenses can come from family medical deductibles, copayments, eye care expenses, prescription drugs, and family dental expenses. It is allowable to exempt medical expenses for yourself, your spouse, and all dependents you list on your federal tax return.

Dependent Care FSA allow dependent care expenses for children under age 14, or a disabled spouse or person of any age who is unable (mentally or physically) to take care of him or herself and who spends at least eight hours a day in your home, to be deducted from your paycheck. The maximum dollar amount allowable under IRS regulations is $5,000 ($2,500 if married and filing separately) per plan year.
