Members of the Building Industry Insurance Trust are insured for Basic and/or Supplemental Group Term Life through USAble Life. If you have completed your enrollment through your employer and are requesting an amount of Supplemental Group Term Life that exceeds $50,000, or are applying outside of your initial 31-day eligibility period, please complete the Evidence of Insurability form below. Once completed, please email your form to membership@usablelife.com for processing. Please refer to your USAble Life summary of benefits for cost information.
